I don't know how to contact him.
He was very strongly against people reprinting his stuff without permission when I knew him (there was a controversy over someone who created a website with word-for-word plagiarism of a wide variety of official AD&D books and fan material), but he had his own website then that he preferred to have control of. But now, over ten years later, he might not care so much.
